During the twenty years of development of the dental profession, porcelain veneers have grown rapidly as a remedy to restore dental aesthetics in dental practices worldwide. They are used extensively in cosmetic restoration of front teeth (front teeth) as well as posterior teeth. The veneer porcelain material has also made great strides, producing high-strength porcelain chisels, which are resistant to fracture and withstand good bending strength but still achieve a minimum thinning capacity to produce veneers.

Ultra-thin veneer can be made in the following cases:

Teeth are narrowed.
Bent teeth
Teeth have many fillings
Teeth are cracked

We know that the root canal is the fibrous tissue in the middle of the tooth including blood vessels and nerves. The root canal and supporting tissues around the teeth provide nutritional support for the teeth, but in some cases root canal treatment is required. Then the surrounding tissues would be responsible for providing nutrition to teeth.Cases when root canal treatment is required:

Inflammation of the tooth pulp or necrosis

Large tooth decay close to root canal

Root canal is exposed because of accidents, or a prosthetic

The steps of root canal treatment are as follows:

Anesthesia (this step does not need to be performed for cases of dead tooth pulp)

Open the entrance to the canal

Clean and taper for the canal

Canal filling using gutta percha

Cleaning and filling canals are important for the success of root canal treament. Previously, K file was used to taper to file the root canal, because the need to use many brooch in order from small to small to large which takes a lot of time, but hard to clean up the pulp, especially in difficult cases, and because the canal cannot be smoothed, the result is sometimes not perfect.

In later years, with the advent of Ni-Ti rotary alignment systems, better cleaning and tapering of the canal was achieved, thus better filling of the canal, and significantly faster treatment time. It can be said that Ni-Ti rotary system improves the quality of dental pulp treatment and increases patient satisfaction.

KFILE – Dr Hung & Associates Dental Center

Picture 01: Treat the pulp with K-File

NITI ROTARY SYSTEM – Dr Hung & Associates Dental Center

Figure 02: Treat the pulp with the Ni-ti rotary broach

Some characteristics of the teeth make the treatment easier or more difficult include:

Bsck teeth are more difficult to treat than the front teeth because of more canals and the available space is more restrictive.

Lower front teeth are front teeth, but the shape of the teeth is thin, canals are often narrow, the treatment of canal is often more difficult than other teeth.

Narrower canals are more difficult to treat than broader ones

Tilted canals are harder to treat than straighter ones

Long canals are more difficult than short canals

Treament through the crown of the crown is harder than without crown

In difficult cases, it is more time-consuming and difficult to treat with K-files because of the length of the canal, the difficulty in removing the pulp and the difficulty in filling the canal.

At Dr Hung & Associates Dental Center, we use Ni-Ti rotary alignment systems including Protaper, Wave-one, Wave-one gold, Reciproc, Hyflex. Most of canal treatments are finished in one appointment, except in some cases of acute myelitis or infection of the root tops that require medication.

The following are pictures of some of the root canal treatment at Dr Hung & Associates Dental Center with Ni-Ti rotary alignment system.

1 / Cure large pulp molars

Case Mr Bonnici:

Cure tooth pulp 17 due to large decay

Ni-ti used: Wave-one

This is the case of upper molars, narrow and curved canals. Using a Wave-one, we prepared the length of his pulp, removed the pulp and created the oblong for the canal, then filled the canal with gutta percha. You can see in the X-ray film after the pulp treatment, the white lines in the teeth are the image of gutta percha used to fill the canal.

2 / Cure large pulp molars

Case Mr Taink:

Cure tooth pulp 47 due to large broken tooth

Nitrile broth used: Reciproc

This is a large pulp molars, with all three canals are bent. We used Reciproc in this case.

3 / Cure through the crown

Case Mr Groza:

Cure tooth pulp 48 through the porcelain crown, due to pulp inflammation

Ni-Ti used: Wave-one

Patients have a porcelain bridge 4 units (from tooth 48 to tooth 45). Due to good bridges, but tooth 48 with painful pulp inflammation, we decided to treat the pulp through the crown to retain the bridges for the patient.

Teeth 48 is the lower wisdom tooth, at the rear end of the jaw, where the tooth 48 is tilted close, and the needle is pierced through the crown, so this is a rather difficult case. Using Wave-one, we have achieved good results

4) Cure the lower mandibular jaw

Case Ms Thao:

Cure tooth pulp 32, 31 and 41 to make dental porcelain aesthetic

Niti used: Hyflex

These are the lower incisors located in the deviating position, designated to have root canal treament to make porcelain aesthetic. This case is treated with Hyflex.

All of the above cases were treated at Dr Hung & Associates Dental Center, completed in one appointment and without pain.

It can be said that the use of Ni-Ti rotary brochure has helped us shorten treatment time, improve the quality and effectiveness of root canal treatment, bring satisfaction and trust to patients.

At Dr.Hung & Associates Dental Center, we ensure complete safety of patients, staff and the hygiene of tools used. We are highly careful about hygiene management. We believe a clean and safe environment takes an inevitably important part in the success of a patient’s treatment.

First, we make sure dental offices are generally clean and in order, free of dust on countertops, work surfaces, chairs and other equipment/furniture. Beside general cleanliness, for patient safety, examination and treatment rooms are cleaned and decontaminated on all surfaces between patients including items such as dental chairs, light, drawer handles and counter tops.

We also employ following procedures and protocols to ensure safety of our staff and patients

Hand hygiene and use of gloves
Protective clothing
Sterilization and disinfection of instruments
Cleaning of dental handpieces (drills)
Use of disposable items
Disposal of waster materials including sharp items
INSTRUMENT STERILIZATION

Dental problems are never fun, but there is good news: Most of them are easily preventable. Brushing twice a day, flossing daily, eating properly and having regular dental check-ups are important in preventing dental problems. Educating yourself about common dental problems and their causes can also go a long way in prevention.

Bad breath

Bad breath (also called halitosis) can be downright embarrassing. According to dental studies, about 85% of people with persistent bad breath have a dental condition that is to blame. Gum disease, cavities, oral cancer, dry mouth and bacteria on the tongue are the reasons that can cause bad breath. Using mouthwash to cover up bad breath when a dental problem is present will only mask the odor and not cure it. If you suffer from chronic bad breath, visit your dentist to rule out any of these problems.

Ways to prevent bad breath:

Brushing your teeth and flossing twice a day, especially after meals that contain food and spices known to cause bad breath. Remember to brush your tongue, or try using a tongue scraper, to remove any trapped food and plaque caught in the tiny hair-like fibers on the tongue. Finish off your brushing by rinsing thoroughly with water or mouthwash.
Visit your dentist on a regular basis (every six months or as indicated by your dentist) for a complete examination of your teeth and gums and thorough cleaning by the dental hygienist.
Preventing bad breath is achievable when you have dental problems treated as they occur, such as tooth decay, gum abscesses, and abscessed teeth.

Tooth decay

Did you know tooth decay, also known as cavities, is the second most prevalent disease. Tooth decay occurs when plague, the sticky substance that forms on teeth, combines with the sugars and / or starches of the food we eat. This combination produces acids that attack tooth enamel. The best way to prevent tooth decay is by brushing twice a day, flossing daily and going to your regular dental check-ups. Eating healthy foods and avoiding snacks and drinks that are high in sugar are also ways to prevent decay.

Gum (Periodontal) Disease

Studies have shown that periodontal disease, also known as gum disease, is linked to heart attacks and strokes. Gum disease is an infection in the gums surrounding the teeth. Gum disease is also one of the main causes of tooth loss among adults. There are two major stages of gum disease: gingivitis and periodontitis. Regular dental check-ups along with brushing at least twice a day and flossing daily play an important role in preventing gum disease.

Oral cancer

Oral cancer is a serious and deadly disease that affects millions of people. In fact, the Oral Cancer Foundation estimates that someone in the United States dies every hour of every day from oral cancer. Over 300,000 new cases of oral cancer are diagnosed every year, worldwide. This serious dental disease, which pertains to the mouth, lips or throat, is often highly curable if diagnosed and treated in the early stages.

Mouth sores

There are several different types of mouth sores and they can be pesky and bothersome. Unless a mouth sore lasts more than two weeks, it is usually nothing to worry about and will disappear on its own. Common mouth sores are canker sores, fever blisters, cold sores, ulcers and thrush.

Tooth erosion

Tooth erosion is the loss of tooth structure and is caused by acid attacking the enamel. Tooth erosion signs and symptoms can range from sensitivity to more severe problems such as cracking. Tooth erosion is more common than people might think, but it can also be easily prevented.

Ways to prevent tooth erosion:

After eating or drinking acidic foods or beverages, rinse your mouth with water so that the acid will be neutralized.Wait at least an hour before brushing your teeth after consuming acidic foods or beverages to allow the teeth to re-mineralize after the acid exposure.
Cut back on carbonated drinks and try replacing them with water, milk or un-sweetened coffee and tea.
Use a straw when drinking carbonated beverages or other sweet drinks, such as natural fruit juices, which are very acidic. A straw enables the liquid to go quickly to the back of your mouth instead of washing over the teeth.
Swallow acidic liquids quickly instead of swishing them around or holding them in your mouth.
Chewing sugar-free gum can help reduce dry mouth and increase the saliva flow, allowing for your teeth to re-mineralize.

Tooth sensitivity

Tooth sensitivity is a common problem that affects millions of people. Basically, tooth sensitivity means experiencing pain or discomfort to your teeth from sweets, cold air, hot drinks, cold drinks or ice cream. Some people with sensitive teeth even experience discomfort from brushing and flossing. The good news is that sensitive teeth can be treated.

Toothaches and Dental emergencies

I can’t think of much worse than suffering from a toothache. While many toothaches and dental emergencies can be easily avoided just by regular visits to the dentist, we all know that accidents can and do happen. Having a dental emergency can be very painful and scary. Fortunately, you can do several things until you are able to see your dentist.

Never put any pain killers, including aspirin, on the gum because it can burn the gum tissue. Clean your mouth out by rinsing thoroughly with warm water. Gently floss around the tooth to remove any food particles that may be caught there. If your tooth continues to hurt, you should call your dentist as soon as possible. A toothache can result from several dental problems. Regular dental check-ups and dental cleanings can help prevent toothaches.

Teeth grinding

Teeth grinding or Bruxism, is very common in adults and can be very destructive, sometimes resulting in jaw problems. The most common causes are misaligned teeth and stress. Orthodontic or restorative dentistry can correct alignment and we can fabricate a custom a plastic guard for people to wear while they sleep to protect their teeth from the destructive forces our jaws can produce. Other prevention methods include physical therapy and daily relaxation techniques.

Unattractive smile

While an unattractive smile is not technically a “dental problem,” it is considered a dental problem by people who are unhappy with their smile and it’s also a major reason that many patients seek dental treatment. An unattractive smile can really lower a person’s self-esteem. Luckily, with today’s technologies and developments, anyone can have a beautiful smile. Whether it’s teeth whitening, dental implants, orthodontics or other cosmetic dental work, chances are that your dentist can give you the smile of your dreams.

Your diet, including the type of food and the frequency that you use them, plays a key role in creating and maintaining your dental health. Sweets have long been known for its impact on teeth, but there are other types of food that can bring a negative effect as well. Lots of food and drinks, especially those contain high levels of sugar, will speed up the process of causing tooth decay.

Taking care of your teeth and gum

Cleaning your teeth regularly is very important. Brush your teeth twice a day and use dental floss or other cleaning tools daily to maintain good oral hygiene. Once the tartar has been formed, they might lead to dental problems and gum bleeding.

You should have regular dental visit to have overall dental health check and clean your teeth properly.

Diet and dental health

Being on diet can reduce the risk of having tooth decay. Drinking products such as soda, juice can create an environment that might lead to tooth decay. Paying attention to the amount of sugar in your diet will also help to protect your teeth. Most types of food contain sugar, including fruit and vegetable, which can potentially lead to tooth decay. You can eliminate this risk by reducing the amount of sugar consumed daily. Otherwise, you should eat sugar-containing products during the main meals rather than afterwards. This is because the amount of saliva produced during main meals is higher, which will have to reduce the amount of acids inside the mouth and remove tiny particles between teeth. Chewing gum is also effective in increasing the amount of saliva produced and also the amount of calcium and phosphate which will enhance your teeth. Chewing sugar-free gum 20 minutes after your meal would prevent tooth decay.

Balanced diet

A balanced diet is very important in maintaining your overall dental health. The United States Department of Agriculture (USDA) has encouraged us to choose food from the following main categories:

• Vegetable, especially those with a dark green or yellowish orange color

• Food made from grains such as brown bread, porridge, etc

• Dairy products such as low-fat yogurt/cheese, etc

• Different types of beans and meat such as chicken, fish or beef

• Vegetable or fish oil

What Is the Right Way to Brush?

Proper brushing takes at least two minutes — that’s right, 120 seconds! Most adults do not come close to brushing that long. To get a feel for the time involved, try using a stopwatch. To properly brush your teeth, use short, gentle strokes, paying extra attention to the gumline, hard-to-reach back teeth and areas around fillings, crowns or other restoration.
Concentrate on thoroughly cleaning each section as follows:

• Clean the outer surfaces of your upper teeth, then your lower teeth

• Clean the inner surfaces of your upper teeth, then your lower teeth

• Clean the chewing surfaces

• For fresher breath, be sure to brush your tongue, too

What Type of Toothbrush Should I Use?

Most dental professionals agree that a soft-bristled brush is best for removing plaque and debris from your teeth. Small-headed brushes are also preferable, since they can better reach all areas of the mouth, including hard-to-reach back teeth. For many, a powered toothbrush is a good alternative. It can do a better job of cleaning teeth, particularly for those who have difficulty brushing or who have limited manual dexterity.

How Often Should I Replace My Toothbrush?

You should replace your toothbrush when it begins to show wear, or every three months, whichever comes first. It is also very important to change toothbrushes after you’ve had a cold, since the bristles can collect germs that can lead to reinfection.

Your oral health is more important than you might realize. Get the facts about how the health of your mouth, teeth and gums can affect your general health.

Did you know that your oral health can offer clues about your overall health — or that problems in your mouth can affect the rest of your body? Understand the intimate connection between oral health and overall health and what you can do to protect yourself.

What’s the connection between oral health and overall health?

Like many areas of the body, your mouth is teeming with bacteria — most of them harmless. Normally the body’s natural defenses and good oral health care, such as daily brushing and flossing, can keep these bacteria under control. However, without proper oral hygiene, bacteria can reach levels that might lead to oral infections, such as tooth decay and gum disease.

In addition, certain medications — such as decongestants, antihistamines, painkillers and diuretics — can reduce saliva flow. Saliva washes away food and neutralizes acids produced by bacteria in the mouth, helping to protect you from microbial invasion or overgrowth that might lead to disease.

Studies also suggest that oral bacteria and the inflammation associated with periodontitis — a severe form of gum disease — might play a role in some diseases. In addition, certain diseases, such as diabetes and HIV/AIDS, can lower the body’s resistance to infection, making oral health problems more severe.

What conditions may be linked to oral health?

Your oral health might affect, be affected by, or contribute to various diseases and conditions, including:

• Endocarditis. Endocarditis is an infection of the inner lining of your heart (endocardium). Endocarditis typically occurs when bacteria or other germs from another part of your body, such as your mouth, spread through your bloodstream and attach to damaged areas in your heart.

• Cardiovascular disease. Some research suggests that heart disease, clogged arteries and stroke might be linked to the inflammation and infections that oral bacteria can cause.

• Pregnancy and birth. Periodontitis has been linked to premature birth and low birth weight.

• Diabetes. Diabetes reduces the body’s resistance to infection — putting the gums at risk. Gum disease appears to be more frequent and severe among people who have diabetes. Research shows that people who have gum disease have a harder time controlling their blood sugar levels.

• HIV/AIDS. Oral problems, such as painful mucosal lesions, are common in people who have HIV/AIDS.

• Osteoporosis. Osteoporosis — which causes bones to become weak and brittle — might be linked with periodontal bone loss and tooth loss.

• Alzheimer’s disease. Tooth loss before age 35 might be a risk factor for Alzheimer’s disease.

• Other conditions. Other conditions that might be linked to oral health include Sjogren’s syndrome — an immune system disorder that causes dry mouth — and eating disorders. Because of these potential links, be sure to tell your dentist if you’re taking any medications or have had any changes in your overall health — especially if you’ve had any recent illnesses or you have a chronic condition, such as diabetes.

How can I protect my oral health?

To protect your oral health, practice good oral hygiene every day.

For example:

• Brush your teeth at least twice a day.

• Floss daily.

• Eat a healthy diet and limit between-meal snacks.

• Replace your toothbrush every three to four months or sooner if bristles are frayed.

• Schedule regular dental checkups. Also, contact your dentist as soon as an oral health problem arises. Remember, taking care of your oral health is an investment in your overall health.

What is good oral hygiene?

A person with good oral hygiene habits will have clean teeth without a bad smell. This means:

• Your teeth does not have food remained.

• The gum is pink and healthy – not damaged or bleeds when brushing or using dental floss.

• If your gum is damaged whilst using dental floss or brushing, or if you have a bad breath, please visit the dentist immediately to have your teeth taken care of.

• Dentists will explain in details how to take care of your teeth, remove the tartar and whiten your teeth, in order to help you to have a beautiful smile with healthy teeth.

How to have good oral hygiene?

• Brush your teeth carefully twice a day and use dental floss daily.

• Have a balanced diet and avoid junk food between meals.

• Use dental products that contain flour, including toothpaste.

• Use mouthwash liquid that contains flour.

Brush your teeth correctly

Tilt the brush at a 45° angle against the gumline and sweep or roll the brush away from the gumline.

Gently brush the outside, inside and chewing surface of each tooth using short back-and-forth strokes.

Gently brush your tongue to remove bacteria and freshen breath.

Use dental floss correctly

Cut off a section of floss about 45cm (18 inches) long. Wind it around the middle fingers of each hand.

Grip the floss with your thumb and forefinger of each hand so that the floss between your hands is about 2.5-5cm (1-2 inches) long. Pull it tight so you can insert the floss in a gap between two teeth.

Gently guide the floss up and down against the sides of the teeth and under the gumline. When the floss reaches the gumline, curve it into a C-shape against the tooth until you feel resistance.

Hold the floss against the tooth and gently scrape the side over it, away from the gum. This will help you to floss under the gumline without causing any damage.

Repeat this process for the next gap, along the side of the next tooth.

Make sure you floss between every single tooth. Working to a pattern round the whole of your mouth makes it less likely that you’ll miss out any teeth.

• Remember to floss the side of the teeth at the back of your mouth.

A- Reasons There are 2 main reasons behind this:

1. Changes in hormones can make the gum inflamed easily. The common symptoms are swollen and bleeding gum.

2. Changes in diet – pregnant women usually have nausea and changes in eating habits, which can potentially lead to tooth decay.

B- Why protect your teeth during pregnancy

Dental diseases if left untreated will weaken your teeth and might eventually lead to tooth loss. According to a medical research in America, pregnant women’s gum diseases can lead to the production of prostaglandins which stimulates labor. Severe infected cases which require medicine might also have an impact on the fetus.

C- Dental care during pregnancy

Women need to take care of dental diseases prior to pregnancy. During pregnancy, women need to be supplied with all necessary nutrients, avoid high-sugar products and eat vegetable and food.

Pregnant women need to have good oral hygiene to prevent tooth decay or gingivitis. Regular dental visit is necessary so that all essential dental work can be done. This should be done between fourth and seventh month of the pregnancy. Pregnant women should only use drugs under doctors’ advice.
